You could ask the individual themselves this is the most effective way and it encourages the individual to take control. You could read the care plans which can give you information if the individual canââ¬â¢t communicate their needs the disadvantages are that the care plan may be out of date and need reviewing, it might not be accurate or may have been written with out much involvement from the individual.
